At Cleveland Nursing and Rehabilitation Center, LLC, I can truly say we have some dedicated and caring staff that do their very best to promote resident’s quality of life. It is our mission to not only provide care for the elderly, but to grow together as a family. It has been a great honor to be able to work, play, encourage, and rejoice with the amazing staff and residents.

Having trouble adapting at home or recovering from a sports injury? We offer outpatient Physical, Occupational, & Speech Therapies. Available for transitional, outpatient, and long term care patients.

Memory Care
Memories Unit
The Cleveland Memories Unit is an intimate, 26-bed unit that provides loving care for both male and female residents who have been diagnosed with Dementia related symptoms.
Our safe and secured unit has been designed to meet the special needs of residents with Alzheimer’s and related disorders. Residents with Dementia related diseases require sensitive care over the progressive course of their illness. We recognize the unique individual needs of each resident who is experiencing memory loss, impairment of functional abilities and other cognitive skills.
Memory Enhancement Training
While there is no cure today for Dementia related disorders, research is continually being conducted to find ways to extend the quality of life for those with the long term debilitating disease. Memory enhancement training with a focus on conditioning the long term memory can assist the resident with daily tasks requiring recall, repetition and naming.
Our professionals have been highly trained to provide compassionate care for the unique needs of our residents. Programming provides emotional and therapeutic activities to maintain the remaining strengths and abilities of each resident. Special emphasis is placed on recognizing individuality and contributing to the resident’s self worth.
Transitional Care Center
Short and Long Term Rehabilitation Center
Our rehab center is staffed with a dynamic and caring team of clinical professionals that work in unison with licensed physical, occupational and speech therapists.
We offer a variety of comprehensive rehabilitation programs focusing on post hospital recovery, orthopedic, neurologic, pulmonary and other medically complex conditions that allow patients to return to their previous level of functioning following an injury, illness or accident.
